123FormBuilder 2FA: how to set up an authenticator app
By the Novaz team · Updated October 29, 2026
Novaz steps checked against version 1.0.5
On this page
123FormBuilder is an online form builder for surveys, order forms, registrations and the like, so your account can hold every submission people have sent you. Out of the box you sign in with a username and password only. Two-factor authentication (2FA) is an option you turn on yourself, and 123FormBuilder calls it Multi-Factor Authentication Device (123FormBuilder). The one method its help describes is an authenticator app that makes a new 6-digit code every so often. This guide uses Authenticator App - Novaz on iPhone. Any authenticator app works the same way.
How we checked this guide: the 123FormBuilder steps come from 123FormBuilder's own knowledge base, linked below, which we read on October 21, 2026. We haven't tested them on a 123FormBuilder account, so a label may differ slightly. The Novaz steps are checked against the app, version 1.0.5.
Where the setting is
The setting doesn't sit in a security tab of its own. You find it in My Account, inside the Change Password section (123FormBuilder). 123FormBuilder's HIPAA page also points to My Account for multi-factor authentication (123FormBuilder). The setup article was last updated in July 2024, so the screens you see today may look a little different.
Turn on 2FA with an authenticator app
123FormBuilder describes these steps (123FormBuilder):
- Sign in to 123FormBuilder and open My Account.
- Go to Change Password, then Multi-Factor Authentication Device, and click Add now.
- A QR code appears on the screen. In Novaz, tap +, then Scan QR code, and point the camera at it. If you're working on the same iPhone, take a screenshot, tap +, then Choose a photo. See scanning a QR code on the same iPhone.
- Novaz adds the account. 123FormBuilder says your app will show your account email with part of it hidden, and a 6-digit code that changes on a timer.
- Back on Multi-Factor Authentication Device, type the current code from Novaz into the text box above Activate Code, then click that button.
123FormBuilder's article only talks about scanning. It doesn't mention a text key you could type instead. If you do see one next to the QR code, you can tap +, then Enter key manually, and paste it as the Setup key. See What is a 2FA setup key? You only scan once: after that, the code is waiting in the app.
The article recommends Google Authenticator, and it names desktop options for people without a smartphone. Novaz, or any other app that makes time-based codes, works the same way.
Signing in afterwards
Once it's on, every sign-in has two steps: your username and password first, then the 6-digit code from Novaz (123FormBuilder). The help doesn't describe a way to mark a computer as trusted and skip the code.
123FormBuilder also lets you sign in with a connected Google or Facebook account (123FormBuilder). Its pages don't say whether the code is also asked for when you sign in that way.
No backup codes in the help
123FormBuilder's help doesn't mention backup or recovery codes, and it doesn't name a second method such as text messages. Novaz keeps its keys on your iPhone, so make a Novaz backup: iCloud backup (PRO) or Export to file… (free) under Settings › Backup. Without one, that iPhone is your only way to make codes. Keep it safe, and know how you'd reach support (below) before you need to.
If 123FormBuilder rejects your code
123FormBuilder's help doesn't cover rejected codes. The usual cause is a phone clock that's off: on iPhone, go to Settings > General > Date & Time and turn on Set Automatically. Novaz warns you when the clock is 15 seconds or more off. Also make sure you're typing the newest code, not one that's about to change. More causes are in Authenticator code not working?
Changing phones
The help doesn't describe moving 2FA to a new phone. What it does document is the off switch and the setup itself, so you can do the move yourself while the old iPhone still works: turn 2FA off, then click Add now again and scan the new QR code with the new phone. If you have a Novaz iCloud backup or backup file, you can restore the account on the new iPhone instead and skip this. Without one, this step is needed. See How to transfer your authenticator app to a new iPhone.
If you lose your authenticator
123FormBuilder's help has no article on losing an authenticator, and it doesn't say what proof it asks for or how long a reset takes. The route it offers for sign-in trouble in general is its support team. For a forgotten username, it asks you to write from the email address on your account (123FormBuilder). Doing the same here should help support find your account. You can reach them through the Contact Us page.

Turning 2FA off
Go back to My Account > Change Password > Multi-Factor Authentication Device and click Disable (123FormBuilder). Then remove the old entry from Novaz: open it and tap Remove account.
